Sekkei感染症対策のチームアプローチ日本の医療現場では、年間17,000人以上が、院内感染が原因で亡くなっていると言われています。また、入院患者の約10％に院内感染が発生している病院もあるとの報告もあります。未知なる感染症が蔓延する現在において、院内感染を防ぎ、患者と医療従事者の安全を守ることは、重要かつ喫緊の課題です。 また、抗生物質耐性菌などの出現により、抗菌薬のみに頼った予防や感染拡大防止だけでは不十分とされ、多面的なアプローチを図る研究活動が展開されるようになりました。医療従事者や研究者、建築設計者による建築分野での感染症対策もそのひとつです。─「感染経路」を断つことに貢献する建築デザイン感染は、「病原体（感染症を起こす微生物側）」「宿主（受け入れる人間側）」「感染経路（病原体を運ぶ伝搬経路）」の3つの要素によって起こります。そして、病院は、この3要素が揃ったリスクの高い場所です。建築デザインにおける対策は、病院内における3要素のうちの「感染経路」を断ち、感染症の拡大を未然に防ぐことに主眼をおいた取り組みです。また、感染経路には、「接触」「飛沫」「空気」の3種類があり、それぞれに応じた対策を講じます［1］。 病院の計画・設計・管理のガイドライン*1では、病院の各部屋を5段階に分類し、空気清浄度が規定されています［2］。病院内を適切に空調ゾーニング（系統を区分け）し、規定の空気清浄度を維持しながら気流を制御することが衛生的に保つことにつながります。 ここでは必要なプランニングやそのディテールにおける感染制御のポイントを簡単に説明します。─①安全な空気の流れを創る：空気感染隔離室（AIIR）感染症対策においては、空気感染に備えて「空気圧」「空気清浄度」「気流方法」「気流分布」の視点で、安全な空気の流れをデザインします。 空気感染症患者のいる「空気感染隔離室」から絶対に空気を漏らさないために、病室への一定の空気の流れをつくり-2.5Paの陰圧を保つ、換気回数の確保、内装の気密仕様、汚れた空気の外部への直接排気、室内の空気循環のための超高性能エアフィルタ使用、陰圧状況の継続モニタリング、陰圧管理に有効な前室の設置、などがポイントとして挙げられます。─②安全な空気の流れを創る：防護環境（PE）免疫力の低下した患者のための病室は、空気感染隔離室とは逆の空気の流れにする必要があります。すなわち汚れた外部空気を絶対に病室内に入れないということです。そのためには、病室を+2.5PaMultifaceted Hospitals are high-risk areas where all three factors are present. Architectural design measures focus on cutting off the transmission routes, thereby helping to pre-vent the spread of infectious disease ［1］. The “Guidelines for Designing and Managing Hospital HVAC Equipment”*1 classifies hospital facilities into 5 cat-egories and prescribes the degree of air cleanliness for each category ［2］. Proper HVAC zoning in hospitals (separated sys-tems) and control of airflow while maintaining the prescribed air cleanliness are ways to sustain sanitary environments.Planning requirements and important points concerning infection control are summarized below.─1. Create Safe Air Flow with Airborne Infection Isolation Rooms (AIIR) For effective infection control measures, safe airflow designs must consider air pressure, air cleanliness, airflow method, and airflow distribution.
